• Receive income support

• Receive tax credit and are named on a valid NHS tax credit exemption certificate
• Are named on a valid HC2 certificate
• Are registered blind/partially sighted
• Suffer from diabetes/glaucoma
• Are considered to be at risk of glaucoma
• Are 40 or over and are the parent/brother/sister/child who has or had glaucoma
• Have been prescribed complex lenses under the NHS optical voucher scheme
